The GS-13 Social Work Program Coordinator is an advance practice social worker who coordinates and implements outpatient Mental Health (MH) programs at the Milo C. Huempfner Health Care Center (HCC) and Cleveland VA Clinic which is organizationally aligned within the Northeastern Wisconsin Outpatient Clinic Division of the MH Division of the Clement J. Zablocki VA Medical Center (VAMC) in Milwaukee, WI, a complexity level 1a facility. Major Duties:
Oversees the administrative and clinical aspects of a major specialty treatment program, such as, but not limited to:
Post Traumatic Stress Disorder, Substance Abuse Treatment Program, Mental Health Intensive Case Management. Responsible for all phases of service delivery, resource and fiscal management, monitoring control points, developing the annual budget, operating within that budget, and accounting for appropriated funds. Administratively responsible for the programming and operations of the outpatient MH clinic, develops policies and procedures, and prepares reports and statistics for facility, VISN, and national use. Responsible for program management with line authority and accountable for all aspects of the clinical care and support services provided to Veterans, Caregivers, and families served in the outpatient MH programs. Develops short and long range goals and plans in collaboration with the Mental Health Division interprofessional leadership team. Develops and implements policies and procedures to ensure goal attainment, management of administrative and programmatic resources, and outcome monitoring using data driven quality assurance processes. Develops outpatient MH services in accordance with national guidelines such as the Uniform Services Handbook, establishes policies and procedures, oversees program development, and directs and clinically oversees outpatient MH providers. Concerted efforts toward collaboration with other VA programs and services like acute psychiatric and primary care services, suicide prevention, and homeless programs, as well as forging external community partnerships. Reviews, monitors, analyzes, plans and assesses outpatient mental health services. Responsible for key project activities related to the planning, development, and implementation of the project component. Develops and implements information systems to track service activities including visits, referrals, inpatient days, patient demographics, equipment utilization and patient satisfaction. Develops implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services. Performs a wide range of services and provides technical assistance to the medical center, state, local, and private agencies, and universities. Assesses current and potential employees' qualifications, selects new employees, advocates for new positions to support the programs' needs, evaluates the performance of employees, and rewards and/or disciplines employees as appropriate. Establishes and maintains effective channels of communication with numerous and varied programs and divisions. Serves as a member of various councils, committees and workgroups as designated, and participates in department/professional continuing education activities and peer review process. Performs advance practice clinical social work functions and specialized clinical social work functions. Establishes and maintains effective therapeutic relationships with Veterans. Performs case finding functions and outreach independently, assessing and screening patients for psychosocial needs. Coordinates community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs. Identifies high-risk patients and provides case management services. Provides crisis intervention services. Maintains complete, accurate, timely and confidential medical record documentation of patient information according to VA, TJC, and CARF policies/regulations. Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j, this part. Education:
Must have a Master's degree in Social Work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. (TRANSCRIPTS REQUIRED) Licensure:
Must be licensed or certified by a state to independently practice social work at the advanced practice level. (COPY OF LICENSE REQUIRED) GS-13 Grade Determination:
One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
Skill in coordinating and implementing one or more specialty program(s), across the facility. Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to collaborate with internal and external partners to further program goals and enhance patient centered care. Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action. Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget. Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies.
