Registered Nurse (RN) --- Intermittent

There is an opening for an intermittent Registered Nurse (RN) (NON-Benefit position)in the Primary Care Clinics at the Northeast Wisconsin Ambulatory Clinic in Cleveland or Green Bay. The function of the Patient Aligned Care team (PACT) RN Care Manager is to deliver evidence-based practice and patient driven holistic care. The RN is responsible for applying the PACT model principles by using the nursing process and evidence-based research within their daily practice. The functions include, but are not limited to, patient assessments, medication administration, patient education, assisting in patient care in emergency situations, telephone and walk in triage, two- and seven-day post hospital discharge calls, and documentation within the computerized patient record system (CPRS). The positions support the Primary Care Patient Aligned Care Team(PACT) Model. The RN Care Manager is a member of PACT and will function as an integral member of the interdisciplinary team in managing chronic diseases. The RN must demonstrate outstanding communication skills with the ability to triage patient concerns and complaints in person or via telephone. The RN in the role of Care Manager applies the principles of advanced clinic access and PACT principles within their daily work. The RN demonstrates advanced clinical expertise in the nursing process (assessment, diagnosis, planning, implementing, and evaluation) when caring for the veteran population. The RN possesses broad-based clinical knowledge and skills to effectively apply all aspects of the nursing process within a collaborative, interdisciplinary practice setting. The RN effectively implements activities towards health promotion and disease prevention along with the management of acute and chronic illness. Work Schedule:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
