Registered Nurse (RN) -- HUD VASH Clinical Coordinator

There is a RN- Clinical Coordinator position available in the Housing and Urban Development-VA Supported Housing (HUD-VASH) Program in the Mental Health Division. The RN- Clinical Coordinator is responsible for providing treatment to patients in the Housing and Urban Development/VA Supportive Housing program. The Incumbent is responsible for monitoring and administration, serving as a primary contact with the local Public Housing Authority (PHA's) and direct patient care (directly or through referral) necessary to ensure that patient care is coordinated with VA Medical Centers, community health or mental health providers and agencies that serve homeless populations. Incumbent prepares reports and statistics for facility, VISN, and national use. Incumbent manages and controls the use of data related to this program regarding day to day functioning and program evaluation and, in conjunction with the Program Manager, plans and organizes patient care activities.
Responsibilities:
Administers information and analytical systems to evaluate and enhance the quality of service provided to patients in the HUD-VASH program. Provides direct care to Veterans who are experiencing homelessness or who are at risk for homelessness, on site at the VAMC as well as in the community. Acts as RN consultant to members of the HUD VASH interdisciplinary team, partnering with the team to provide nursing assessment and assist in developing treatment plans for Veterans in the HUD VASH Program. Collaboration with various community agencies, such as the Public Housing Authority, Veterans Service Organizations, and other local social service providers. Develops and implements information systems to track service activities including visits, referrals, inpatient days, patient demographics, equipment utilization and patient satisfaction. Develops, implements, and evaluates clinical guidelines and protocols to establish appropriate utilization of services. Provides technical assistance on monitoring activities, program reviews, and the development/analysis of information to the Medical Center, State, local, private agencies, and universities. Interprets trends and patterns, applies guidelines, and recommends program enhancements. Assesses, plans, implements, reviews, and monitors HUD/VASH program. The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. Excellent interpersonal skills Ability to function independently without direct supervision 3-5 years of clinical nursing experience preferred Mental Health experience preferred MSN preferred Current certification in BLS/CPR Experience working with multidisciplinary teams Knowledge of computerized processing operations and procedures and is competent in word processing, data management programs and other relevant computer applications. The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
