Registered Nurse (RN) -- Caregiver Support Program RN

There is an immediate opening for a Registered Nurse-Caregiver Support RN. This is a new position within the Social Service Department with a duty location of Green Bay, WI. The Caregiver Support Program Nurse (CSP RN) is a registered nurse who provides clinical evidence-based services and interventions for Veterans and their Family Caregiver(s) as well as serving as a resource to staff and community partners in addressing issues faced by those caring for Veterans. Responsibilities include conducting Veteran-centered assessments with Veterans and their Caregivers in the home environment. These assessments include a safety evaluation of the home, physical and psychosocial assessments of both the Veteran and his/her Caregiver, along with monitoring the Veteran's compliance with treatment recommendations to promote his/her attainment of the highest possible level of independence. The CSP RN implements the Caregiver Support Program specific to his/her area of assignment and ensures compliance and collaboration with local VAMC and the VHA Central Office Caregiver Support Program initiatives. The incumbent will function in support of Veterans and their family caregivers throughout the medical center and in community settings. The incumbent will conduct wellness visits to Veterans and Family Caregivers approved in the program and will provide assessment via phone and clinical video telehealth. The incumbent is responsible for developing and establishing linkages and collaborate on VA caregiver support efforts with community healthcare and social service agencies. The incumbent possesses advanced knowledge and understanding of the services and benefits available for caregivers from both federal, state and private sector resources. Work Schedule:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Experience:
A Certification in Ambulatory Care. Possess the skills needed to effectively work with others to communicate directions and offer assistance. The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
